If you wish to transfer your child’s care to Children’s Medical Group, please give us a call. We are happy to walk you through the process. For every new patient, we will need an up-to-date copy of their immunization record. If your child has any past medical history or ongoing medical conditions, we ask that you request their medical record to be transferred to us from their existing physician using this form.
